Rates vary based on time of Assessment.
Rates for administration, scoring and report range from $280 to $420
Therapy Sessions
Each Speech Therapy session is 30 minutes in duration . The rate per $55 per session.
Occupational Therapy sessions are 45 minutes in duration. The rate per session is $70
Private pay sessions are payable by cash, check, Venmo and PayPal.
ENROLLMENT FORMS FOR CLIENTS
Prior to your first appointment, please complete the attached forms and either fax them to the site at which your appointment is scheduled or email them to seasiderehab@gmail.com. In addition to these forms, please be sure to include or bring with you the day of the appointment, a prescription from your physician stating “Speech/ Language Evaluation and Treatment”, “Occupational Therapy Evaluation and Treatment”, or “Dysphagia Evaluation and Treatment”. Also be sure obtain a referral if your health plan requires one. A copy of the front and back of your insurance card will also be required. I look forward to meeting you.
